Community Health Worker – Central Plateau

About the role

The Community Health Worker will conduct home visits in the Central Plateau region to identify pregnant and lactating women and children under five, screen for acute malnutrition, refer cases to health facilities, and support mothers through education and community sensitisation.

Key responsibilities

• Carry out home visits to identify pregnant women, lactating mothers and children under five according to the work plan.
• Screen children aged 6‑59 months for acute malnutrition using MUAC and check for nutritional oedema.
• Refer identified malnourished children and at‑risk pregnant women promptly to appropriate health structures.
• Follow up referred cases through admission, treatment and recovery according to national protocols.
• Train and mentor mother leaders, support groups and community relais on health, nutrition and hygiene best practices.
• Conduct community awareness sessions on exclusive breastfeeding, infant and young child feeding, hygiene and disease prevention.
• Promote essential family health practices related to maternal, neonatal and child health.
• Support vaccination, vitamin‑A supplementation, de‑worming and other public‑health campaigns.
• Report suspected disease outbreaks or health emergencies to supervisors and authorities.
• Collect, compile and transmit health and nutrition data using approved tools and platforms.
• Participate in community meetings, coordination sessions with health structures and project planning workshops.

Required profile

• Experience in community health or similar field work.
• Ability to conduct household visits and identify target groups.
• Knowledge of MUAC measurement and nutrition screening protocols.
• Capability to refer and follow up cases through health facilities.
• Skills in training, coaching and community sensitisation.
• Aptitude for accurate data collection and reporting.
• Good communication with supervisors, health authorities and community members.
