Community Health Worker (20) – Nord

About the role

The position supports vulnerable children and families by providing community‑based health, nutrition and hygiene services. You will work directly in targeted communities to identify pregnant women, lactating mothers and children under five, and ensure they receive timely care.

Key responsibilities

• Conduct home visits to identify pregnant women, lactating mothers and children under five; record households visited and individuals identified.
• Screen children aged 6‑59 months for acute malnutrition using MUAC and edema assessment; report screened children and identified cases.
• Refer malnourished children and at‑risk pregnant women to appropriate health facilities; track referral completion.
• Follow up referred cases through admission, treatment and recovery according to national protocols.
• Train and support mother leaders, support groups and community relais on health, nutrition and hygiene best practices.
• Lead community awareness sessions on exclusive breastfeeding, infant and young child feeding, hygiene and disease prevention.
• Promote essential family practices related to maternal, neonatal and infant health.
• Participate in vaccination, vitamin A supplementation, deworming and other public‑health campaigns.
• Identify and promptly report priority disease cases, epidemics or health emergencies.
• Collect, compile and transmit health and nutrition data using approved tools and platforms.
• Attend community meetings, coordination meetings with health structures and project planning sessions.
