About the role
We are seeking a Community Health Worker to join our mission of supporting vulnerable children and families in the Nord‑Est region. The role involves conducting home visits, identifying pregnant and lactating women and children under five, and linking them to essential health and nutrition services.
Key responsibilities
- Conduct home visits to identify pregnant and lactating women and children under five according to the work plan.
- Screen children aged 6‑59 months for acute malnutrition using MUAC and check for nutritional edema.
- Refer identified malnourished children and at‑risk pregnant women promptly to appropriate health facilities.
- Follow up referred cases through admission, treatment and recovery, ensuring compliance with national protocols.
- Train and support mother‑leaders, support groups and community relays on health, nutrition and hygiene best practices.
- Lead community awareness sessions on exclusive breastfeeding, infant and young child feeding, hygiene and disease prevention.
- Promote essential family health practices related to maternal, neonatal and child health.
- Participate in vaccination, vitamin‑A supplementation, de‑worming and other public‑health campaigns.
- Detect and report priority disease cases, epidemics or health emergencies to supervisors and authorities.
- Collect, compile and transmit health and nutrition data using approved community tools and platforms.
- Attend community meetings, coordination sessions with health structures and project planning workshops.
Required profile
- Strong commitment to improving the lives of vulnerable children and families.
- Ability to work independently in community settings and travel to remote households.
- Good communication skills for health education and community mobilization.
Required skills
- MUAC measurement and nutritional edema detection.
- Community health and nutrition screening techniques.
- Referral and case‑follow‑up procedures aligned with national protocols.
- Data collection, reporting and use of community health information tools.
